&lt;p&gt;You&#039;ve removed the item from every computer that accesses the mail account? I have yet to mine come back since removing the item from both computers I use back in early December. I&#039;ve tested with making some todos in iCal. Creating a new To Do in Mail.app however would create one locally but after deleting the offending todo and restarting Mail.app it was gone - it did not create new ToDo.mailboxes&lt;/p&gt;
